·  If you are a domestic student taking a co-op placement, and are interested in our extended Health and Dental coverage during your co-op term, please complete the following application and make sure you have a copy of your co-op registration as it is one of the

*International Students’ family/dependent(s) must have the basic medical services offered by We Speak Student (Industrial Alliance) in order to be eligible for the extended Health and Dental plan. Please complete the family/dependent Coverage Application form for We Speak Student (Industrial Alliance) at any Student Association.

·  Students who are registered with a modified course load and considered full-time; and/or are registered with the Accessible Learning Services, please complete the following application and make sure you obtain a letter from your Accessibility Consultant confirming you are taking a modified course load and  are considered a full-time student as it is one of the requirements for you to successfully opt-in to the plan : NOTE: Health Benefit Premium Fee is $131.40 and Deadline is Feb. 9th, 2017

Modified Course Load Opt-In Online Application

·  If you lost your alternative benefits plan coverage, and would like to reinstate the student Health and Dental Plan, please complete the following application and make sure you get a termination letter from your previous insurance provider as it is one of the requirements for you to successfully opt-in to the plan: NOTE: Domestic Health Benefit Premium Fee is $131.40 and Post-Secondary International Health Benefit Premium Fee is $388.28 and Deadline is Feb. 9th, 2017

Rescind Waiver Online Application

·  If you are registered under the Transitions to Post-Secondary Education (TPE) program, please complete the following application and make sure to get a letter from your T.P.E Support Services coordinator confirming that you are taking a TPE program as it is one of the requirements for you to successfully opt-in to the plan: NOTE: Health Benefit Premium Fee is $131.40 and Deadline is Feb. 9th, 2017
